*Receipt of completed forms/packets is required before an appointment can be scheduled. Once Dr. Papka receives the completed packet, you will be contacted to set up an appointment.
If the Neuropsychological or Functional Assessment will be part of a comprehensive assessment at the Cognitive and Memory Disorders Program at Atlantic Neuroscience, the evaluation with Dr. Papka can occur before, after, or on the same day as the Neurological Evaluation. The scheduling will depend on availability and preferences.
